SGSA Player Clinic Enrollment Waiver Form
You only need to provide a waiver for your player ONCE to participate in all of the free winter clinics!!
All players attending the free winter softball clinics will be required to fill out and sign the Enrollment Waiver form. This must be done prior to participating in any SGSA clinic. Please click on the document link below, print the form, fill it out, bring it to the first clinic session attended.
